Silver Key Senior Services Social Services Case Manager Description: This position is responsible for the management of Silver Key Social Service cases, providing assessment of need and assistance to seniors presenting to Silver Key. Essential Duties and Responsibilities: Case Management including but not limited to: maintaining a caseload of Silver Key Social Services cases; meeting with clients through home or office visits, or any other appropriate location; providing referral to Silver Key and community based programs and resources and assisting seniors access those programs and resources as necessary; prompt and detailed documentation of cases; continually building a broad knowledge of community resources and services to assist clients; and advocating for seniors within Silver Key and with outside agencies and organizations. Minimum Requirements: Education: BS/BA degree. Considerable, documented experience with increasing level of responsibility may be considered with or in lieu of education. Experience: Social Services or Human Services experience, experience working with seniors, or other background.
